- Completed and Signed Enrollment Application Form PDF
- Signed Confidentiality Agreement PDF
- Signed Disclaimer Agreement PDF
- Copy of your current Curriculum Vitae (CV)
- Proof of malpractice insurance coverage (copy of liability insurance policy)
- Deposit of $2,900 due with application.
